Understanding body image through a narrative lens
When you think about body image, you are also thinking in stories. Those stories may be the short, sharp lines you hear in your head about appearance, worth, and belonging. They may be accounts you inherited from family, school, sport, media or social groups. Narrative therapy begins from the simple idea that these stories shape how you experience your body, rather than being the only way to understand it. The approach, developed in the 1980s in Australia and New Zealand by Michael White and David Epston, encourages you to examine the shape and source of the stories you carry. In practice, that means treating the problem - such as a harsh body image narrative - as something you can talk about and look at, rather than as an identity you must accept without question.
With body image, the familiar story often sounds like a series of fixed rules - what makes you acceptable, safe, or loved. Those rules can feel so natural you forget they are stories. Narrative work invites you to slow down and name the narrative, to notice its language and the moments it leaves out. By doing that you begin to see where the story came from, how it has been kept alive, and where it no longer fits. This shift creates room for different accounts of who you are, ones that reflect your values and commitments and that you can test in everyday life.
How narrative therapy works with body image
Externalising the problem and locating its influence
Narrative approaches commonly begin by externalising the problem - giving the harsh body image story a name so it can be discussed as something affecting you rather than being you. When you put a label on the problem, you and your therapist can describe how it operates - when it is louder, who or what it listens to, and what it expects of you. That separation makes it possible to map patterns without blaming yourself, and to identify the relationships, rules and pressures that keep the story in place.
Tracing origins and seeking unique outcomes
A narrative practitioner will often trace where a familiar body image story originated: which comments, images, or practices taught it to you, and how those influences were reinforced over time. This is not an exercise in assigning fault but in seeing the social and relational roots of a story. Equally important is the search for unique outcomes - moments when the story did not hold true. You may have acted in ways that contradicted the harsh narrative, or felt a small relief in a situation where the story was absent. These exceptions are evidence that other accounts of your body and worth already exist, and they become building blocks for a different narrative.
Other narrative practices that may appear in work on body image include re-authoring conversations, where you and the therapist deliberately craft alternative descriptions of your values and commitments, and re-membering conversations, which attend to who you include in your life story and why. Some therapists use outsider witness practices or definitional ceremonies to let others reflect back the changes they see, but how and whether those practices are used varies by therapist and setting.
What to expect in narrative sessions for body image
If you try a narrative-oriented session, the early conversations will often feel like careful listening and mapping. Your therapist will ask about the language of the problem, and you will spend time describing how the body image story shows up in your life - the phrases it uses, the times it is strongest, and the things it asks you to do or avoid. Questions tend to be open and exploratory rather than prescriptive. You might be invited to name the problem, to describe its effects, and to reflect on small moments when it did not dominate your choices. These early moves are about making the familiar story visible so you can relate to it differently.
As work continues, conversations typically shift toward re-authoring: developing richer descriptions of your values and actions that contradict the old story. This might include exploring actions you have already taken that align with a preferred story, and considering how to make those actions more present in your daily life. Pace is usually collaborative and paced to your comfort; narrative sessions aim to follow what matters to you and to test new accounts gently. Keep in mind that most therapists who list narrative therapy among their approaches combine it with other methods, so the balance between narrative practices and other techniques will differ from practitioner to practitioner. It is reasonable to ask early on what a typical session looks like and how much of the work is narrative in orientation.
Is a narrative approach the right fit for body image?
There is no way to tell in advance whether a particular therapeutic approach will suit you. People who are often drawn to narrative work tend to be those who want to move away from being defined by a diagnosis or a set of symptoms and who want to explore the social and relational stories that have shaped their sense of self. If you feel that family messages, cultural ideals or workplace norms have handed you a thin or unkind story about your body, narrative practice offers ways to examine and rewrite that account. The work values your agency and treats you as the author of your life rather than as a passive recipient of interventions.
Compared with approaches that focus primarily on thought content or skills training, such as cognitive-behavioural or skills-based therapies, narrative work gives more emphasis to the contextual origins and cultural shape of body image stories. Where psychodynamic work explores unconscious meanings and past relationships in depth, narrative work tends to foreground the stories you can describe and offers collaborative ways to construct alternatives. Many therapists integrate narrative practices with other methods when that seems helpful. The important point is that you cannot know for sure whether narrative therapy will feel right until you try a conversation; many people find an initial session helpful simply to see how the approach fits their way of thinking and relating.
How to choose a therapist who works narratively with body image
When you look through listings, pay attention to the language therapists use to describe their work. If a profile says a clinician "lists narrative therapy among their approaches" or "uses a narrative approach," that indicates an interest in those practices but does not assert any particular credential. You can ask straightforward questions in a first conversation: how much of their work is narrative, what a typical session looks like, how they describe externalising and re-authoring in practice, and which other approaches they commonly combine with narrative methods. It is also reasonable to ask about their licensure or registration in your location, and about the populations and life stages they have worked with.
Fit often comes down to the quality of the first conversation - whether you feel heard, whether the therapist invites your perspective, and whether their style of questioning and reflection aligns with your preferences. Narrative work is conversational and reflective, so it translates readily to video sessions; you can get a clear sense of how a therapist works from an online meeting. Take time to notice whether the therapist asks about the stories that matter to you and whether they treat the problem as something that can be described and changed rather than as an intrinsic flaw. Those are practical markers that the work may lean narratively.
Above all, trust your judgement. If the way a therapist talks about their approach resonates with how you want to explore body image - focusing on stories, origins, exceptions and preferred values - then a narrative conversation may be worth trying. Remember that many therapists blend narrative practices with other approaches, so it is appropriate to ask how those methods are woven together and how they might support the preferred story you want to live into.
