Understanding chronic illness through a narrative lens
When you live with a long-term health condition you also carry stories - about what the illness means, who you have become, and what others expect of you. Narrative therapy begins from the observation that people make sense of their lives through stories, and that those stories are formed by family, workplace, medical culture, and wider social ideas. Many of the stories people with chronic illness inherit are thin - short summaries that emphasise limitation, loss, or a single identity focused on diagnosis. These thin accounts can feel like the only available way to describe yourself, squeezing out complexity, hopes, or values that still matter.
Narrative therapists - a term for clinicians who list narrative therapy among their approaches - aim to map those stories and to create conditions for a richer account of your life. Rather than focusing primarily on symptom lists or on correcting thought patterns, narrative work pays attention to how problems are named and located in story. It treats the illness narrative as something that can be discussed, questioned, and re-shaped over time. This does not promise an outcome or a cure. Instead, it offers a way to examine the meanings that stick to you and to open space for alternative accounts that better reflect your commitments and values.
How narrative therapy works with chronic illness
Narrative practice uses a set of conversational practices that help you separate the problem from your identity, trace where a familiar story came from, and gather moments that do not fit that story. A common early move is externalising - talking about the problem as something separate from you so it can be described, named, and examined. For example, rather than saying I am exhausted and therefore hopeless, you might come to name "exhaustion" as a presence that interacts with you in particular contexts. Naming the problem in this way allows you and the therapist to notice patterns, triggers, and relationships without collapsing you into the issue.
Practitioners also attend to thin and rich description. Thin description reduces events to short labels and leaves out context and meaning. Rich description expands the story - where the belief began, who contributed to it, and how it plays out in everyday life. Narrative therapists look for unique outcomes - instances when your behaviour, response, or experience contradicts the dominant problem story. These moments, however small, become seeds for a preferred story - an account of your values, commitments, and hopes that can hold up in ordinary life.
Other narrative practices you may encounter include re-authoring or re-membering conversations, which invite you to rework your relationship to significant people and to memory; outsider witness practices, where others listen and reflect back how your story lands; and definitional ceremonies, structured conversations that collect and strengthen new meanings. What to expect in narrative sessions for chronic illness
Narrative sessions tend to be conversational and exploratory. Early conversations typically focus on naming and mapping the problem - what it is called, how it operates, and the contexts in which it becomes stronger or weaker. Your therapist may ask detailed questions about particular incidents that do not fit the main story - those unique outcomes - and will invite you to describe the meanings you attach to them. Questions are often open and designed to draw out context, relationships, and values rather than to measure symptom severity.
Over time the pace can shift from inquiry and mapping to re-authoring. Re-authoring is less about teaching coping techniques and more about crafting alternative narratives that make room for agency, preference, and personal values. You might work on telling these new accounts to others, rehearsing different ways of exercising choice, or experimenting with small acts that align with the preferred story. Compared with approaches that work primarily on thought content, such as cognitive methods, narrative therapy shifts the focus from disputing beliefs to tracing where beliefs came from and how they function in your life. Compared with therapies that emphasise skills training - for example, methods that focus mainly on behavioural or emotional regulation - narrative work privileges story and meaning. Compared with psychodynamic approaches that concentrate on insight into the past, narrative therapy emphasises the stories you live by now and how alternate stories might be enacted. Choosing a therapist is a personal decision that combines practical considerations and relational fit. When you look at profiles, compare focus areas, listed approaches, credentials, languages spoken, and relevant experience. If a clinician lists narrative therapy among their approaches, that signals an interest in story-based work but does not indicate a specific credential. It is reasonable to ask in an initial conversation how much of their work is narrative, what a typical session looks like, and how they integrate other approaches when necessary.
In a first call or message you can also ask where they are licensed or registered and what local regulations apply to their practice. If you are considering online sessions, know that narrative work is conversational and translates well to video; many people find that face-to-face conversation on a screen can support the same exploratory processes as in-person work.
